Plan to Correct

When going outside and coming in from outside, head counts will still be completed with the attendance sheet on the clipboard. There will be at least 2 workers outside with the children when 2 workers are needed to maintain ratios. 1 worker will be with them when there are less children and remaining in ratio. If there are 2 workers, one will be in the front of the line & one will be in the back making sure all children are accounted for and make it outside and back inside. If there is only one worker, they will walk backwards watching the children as they go in and out to make sure all children make it in and out. Workers will also now use a walking rope when going outside and back inside. The children that like to wonder off while in line, will now be placed at the front and will also have the walking rope to keep them in line.

Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

We're now using a program Brightwheel app, that gives all staff the ability to contact any and all parents, immediately. All staff have been notified that following an incident, DHS, the director/person in charge, and the parent must be notified immediately. Staff is aware they can communicate with parents through Brightwheel app program.

340:110-3-293(b) - Parent reporting. Parents and personnel communicate daily and when additionally required, per OAC 340:110-3-280(d).

The director stated she had a meeting with staff on 8/25/25 and training with R&R, Active Supervision. The director stated that they have a checklist and physically touch the child and match to the checklist every hour. They also use the checklist when they go outside and again when they come back inside.

Director states that now there is someone here at all times that is over 21 years old and she will continue to do that.

The director states she went over the licensing book again with staff and all staff signed they understand. Director states she will be sure to contact licensing as soon as possible when anything happens that requires reporting within the required time.
