Complaint:
Reporting- Program did not inform parent of injury to infant’s arm while in care that required medical treatment.
Complaint:
General qualifications, responsibilities, and professional development-Abuse by teacher resulting in broken arm of infant, teacher completed a false injury report, teacher inappropriate towards infants by picking up and pulling by arm, pulling by legs, picking up by back of diaper, and did not recognize hazard to physical safety of infant when hair band was down covering eyes, nose and mouth.

Additional Non-Compliance Found During Investigation: Learning program principles- Infants in swing, bouncer, and crib awake more than 20 minute increments.

Child records and documentation -Daily written information was not updated after 11am that day for infant in care.

Additional Non-Compliance Found During Investigation: Rest time-Infants- Blankets placed on sides of cribs with infants in cribs, infants placed on tummy in crib, and infant asleep in bouncer and not moved to crib.
