Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
Licensing observed the owner, responsible entity, or director did not notify licensing of a change in director the next business day. The previous director's last day was reported being over the summer and new director was designated before program resumed. The program resumed care of children on August 15, 2024
Licensing observed children records for six children without documentation of immunizations
Licensing observed children records of one child missing documentation of signed and dated compliance file notification form.
Licensing observed staff no longer employed listed on the OPDR and staff positions not updated.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
