Plan to Correct

On 02/26/2024, the director and owner spoke with all program personnel about the proper way to pick up children under both arms and to not grab children by their arms. A paper about nursemaid elbow will be put in every classroom that will explain what it is and how to prevent it from happening. The teacher will return on 04/08/2024 to the program. The teacher will be in her same classroom with a co-teacher until she is on maternity leave. The teacher will not be left alone with children.

Plan to Correct

Will open another room if needed but will not go over the room capacity of 21 children. During the monitoring visit, 9 school age children were moved to another room.

Regulation Description

340:110-3-301(b)(4) - Classroom capacity. Classrooms are not routinely occupied by more children than can be accommodated by the square footage per (1) of this subsection.

Plan to Correct

Typed by licensing worker: Will make sure that the correct number of teachers to child ratio is in the classroom. If a teacher needs anything or needs to step out of the classroom for any reason, they will need to let the director or staff in charge know. The teacher cannot leave the classroom until the other teacher arrives in the classroom. Program has an electronic attendance application with bright wheel they use daily and up-date as need and reflects the age and number of children assigned to classroom and teacher and teacher will makes sure they aware of the children they are assigned and provide adequate supervision.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

Another teacher will be in the classroom or some children will be moved to another room to maintain ratio. During the monitoring visit, another teacher was added to the room and 10 children left for school.

Regulation Description

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

Plan to Correct

Will make sure all teachers know when an infant does fall asleep they have to be put in their cribs immediately. Will do safe sleep training. All staff will know about what can and can't go in cribs, blankets, sleep sacks, etc.

Regulation Description

340:110-3-296(b)(2)(B) - Inappropriate sleep environment. Infant safe sleep environment requirements listed in (i) through (viii) of this subparagraph are met.

Plan to Correct

Once new hire finishes all training staff handbook, new staff orientation online, safe sleep, ELCCT, and Professional development plan, etc. will be sent over to the centers licensing specialist.

Regulation Description

340:110-3-284(d)(4) - Safe sleep. Prior to caring for infants, personnel obtain formal professional development in safe sleep practices.
