Plan to Correct

Program will ensure teachers and staff know to look at allergy charts upon entering a classroom. Program put allergy charts on breakfast, lunch and snack carts. Program will ensure special/individualized menus posted where substitutes and floaters can see.

Plan to Correct

Program will Implement transition face-to-name sheets to be used anytime children move locations within or outside the facility; Director will have one-on-one conversations with every staff member regarding the policies and procedures regarding supervision.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

Ms. Barnes stated that all pills given will be counted and all medication given will be documented or termination will occur. As an additional plan of correction for medicine distribution, we have created a sign out sheet which contains date/time/child/personnel checking the medication out.

340:110-3-281.4(b)(8) - Medication administration. Each dose administered, excluding diaper cream is immediately documented on an OKDHS or program form containing the same information.

Staff will sit on a bus seat and use a seatbelt properly during transportation.
