Middle back classroom - heater was on window ledge with no staff present in the classroom, with hot surface, and less than three feet from flammable materials; curtains and paper materials were close to heater. South classroom - heater was on top of a table with no staff present in the classroom, with hot surface, and less than three feet from flammable materials; papers and other materials were close to heater.

The only master teacher for the program (also program director) is not present at the center for at least 30 hours per week. Attendance records were checked from 6/20/22 to present; these reflect work hours to be between 0 - 20 hours per week (see discussion).
Teacher employed 2/16/22 has not obtained current CPR/1st aid training; was due by 5/16/22.
Personnel information forms for three staff employed past two weeks have not been submitted to licensing.
Central detection and alarm system was not inspected within 12 months from last inspection; dated 8/05/21; expired 8/05/22.
Fire extinguishers were not inspected and tagged within 12 months from last inspection; date of inspection is July 2021; expired 7/31/22.
