  • The door to one of the classrooms was loosely screwed into place, causing the door to come off the opening when the door was opened or closed.
  • One of the classrooms at the program was out of ratio when Licensing arrived, having 17 children and two teachers. Six of the 17 children were one year olds.
  • Licensing could not verify that the fire extinguishers at the facility were inspected and tagged by a state licensed authority within the last 12 months
  • One of the program's teachers has been employed at the program since October of 2025 and has incomplete results.
