Eight teachers have expired PDL certificates.
Eight teachers obtained less than 12 hours of annual professional development within their PDL certification period to qualify for a current PDL certificate.
Two teachers have expired PDL certificates & have not submitted an application for renewal; 1 other teacher has not submitted a PDL application since being employed for longer than 12 months.
One teacher has closed criminal history results, another teacher did not submit a criminal history review request.
Parent permission was not obtained for a swaddled infant.
One teacher has not obtained CPR/FA certification within 90 days from date of hire & one teacher does not have evidence of having obtained CPR/FA certification within 90 days from date of hire.
Two teachers employed longer than 12 months have not obtained a PDL certificate.
Outdoor multi-tricycle go around has 2 broken seats.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

SERIOUS: Discipline - Teacher pulled child by arm resulting in injury.
Plan to Correct
Due by 07/02/2023. Received 8/22/23--Staff are- and will continue to be- provided a through Orientation within the first week of employment through: cecpd videos, discussion/training, Q&A, observing staff/teachers, shadowing then, ultimately, counted as Ratio in a classroom when comfortable with supervision. To better equip staff, and the appropriate staff observed exhibiting this behavior and others in this document, I am requesting the data that confirms this was, indeed, observed as a Serious non-compliance. My investigation and interviews did not determine the the "Teacher pulled child by the arm resulting in injury" as stated in the Observed Non-Compliance. I have submitted substantial documentation regarding my investigation. Further investigation will be done to determine when the Licensing Worker observed this behavior to merit a Serious Non-Compliance.

340:110-3-288(c)(1)(A) - actions that could cause pain, such as shaking, hitting, striking with an object, spanking, swatting, thumping, pinching, popping, shoving, spatting, hair pulling, yanking, and slamming;

Additional Non-Compliance Found During Investigation: Discipline - Two-year-olds are being placed in time out for more than two minutes.

Due by 07/02/2023. Received 8/22/23. Staff are- and will continue to be- provided a through Orientation within the first week of employment through: cecpd videos, discussion/training, Q&A, Observing Master Teachers, Shadowing then, ultimately, counted as Ration in a classroom when comfortable with supervision. One of the very first things they learn are: redirection, reflection, reframing, refocussing, removing object/or child from situation if safety is concern. This is evidenced in documentation submitted in Grievance.

340:110-3-288(b)(11)(C) - Time-out is limited to one minute or less for each year of age. However, when self-control is regained in less time, the child may rejoin an ongoing activity.

Additional Non-Compliance Found During Investigation: SERIOUS: Discipline - Outdoor play time is being withdrawn as a form of discipline.
Plan to Correct
Due by 07/02/2023. Received 8/22/23. Again, to better equip staff, and the appropriate staff observed exhibiting this behavior and other in this document, I would like the data that confirms this was, indeed, observed as a non-compliance. As leadership, we do not condone this form of discipline and make corrections with staff if inappropriate actions are being taken. The only time that Outdoor playtime would be considered to be withdrawn is when a child has endangered self or others even after redirection has been attempted.
