At the time of licensing's visit, one personnel has not maintained a current Oklahoma Professional Development Ladder within 12 months of employment.
One personnel has not obtained at least the number of professional development clock-hours to maintain a current OPDL certificate at the time of Licensing's visit.
Personnel job positions are not listed correctly on the Direct Care Organization, therefore the program is not maintaining all items listed on the Direct Care Facility Handbook.
Licensing observed two personnel being employed within 12 months not obtaining and maintaining a current OPDL at the time of visit.
Licensing observed 11 staff not obtaining annual infection disease control training at the time of visit.
Licensing observed two personnel not obtaining at least the required number of professional development clock-hours to maintain a current OPDL certificate.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation.
