Plan to Correct

Staff received a written reprimand. Clips on the gate have been replaced. Assistant director were overseeing staff during outside time the next couple of days after the incident to ensure all requirements were being followed regarding supervision and outdoor safety and to ensure the whole playground is being watched at all times. Staff meeting to address outside procedures including positioning staff on the playground and staff asking another staff to relieve them if they need a break sot that supervision is maintained.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

Staf meeting to address outside procedures including using tablets to clock in/out kids to maintain an accurate count of kids, counting kids periodically, positioning staff to see the entire playground, taking kids inside as a whole group for bathroom breaks to maintain ratio. Addressed importance of maintaining ratios and supervision at all times. If staff needs a restroom break they are to message on the app asking for someone that is able to come relieve them.

Regulation Description

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.
