Plan to Correct

Staff have been retrained on active supervision requirements, including positioning, head counts, and anticipating children's movements. Staff will be strategically positioned during activities to reduce risk and improve response time. The Director will conduct routine supervision checks and provide coaching as needed. Staff-to-child ratios will be monitored and maintained at all times.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

The Director has reviewed DHS injury reporting requirements with all staff. All incidents reports will be reviewed by the Director the same day to determine if DHS notification is required. In the future, if we do not know our current licensing worker we will find an OKDHS caseworker and report to them.
