Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
Licensing observed teacher of a two year old child mistreating her on a cot by aggressively pulling her right arm to try and force her to lay down on her cot for rest time Child was noticeably upset and afraid. Teacher was also observed harshly speaking to the child.
Two metal baskets on the playground are missing wheels and have jagged metal edges accessible to children.
Outlets in the school age room, yellow room and three year old room had uncovered outlets that did not have safety devices in them. The purple room had an extension device plugged in to increase outlet capacity that did not have protective covers.
Child enrolled 6/8/28 did not have immunization record or exemption record on file.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation.
