Director does not have current first aid and CPR training; this expired 6/10/2023.
Director does not have a current Oklahoma Director's Credential; silver ODC is expired as of 2/1/2024
Master teacher (also director) has an expired level 7 OPDL. Expired 2/1/2024.
One teaching staff employed with the program since 2019 does not have current first aid and CPR training.
Program director has been caring for an infant and has not completed infant safe sleep training; no record of this could be provided; the infant has been in care at least since August 2023 (enrollment form is not available and enrollment date was not certain).
There is no crib available for an infant that has been attending the program for several months; director stated the infant has been sleeping on a mat.
There was a leak in the ceiling of the building that has not been repaired.
Personnel (director and teacher) transporting children do not have current CPR and first aid training; expired for both.
Child passenger restraint systems (booster seats) are not in place or available for five children under age eight who are being transported in an SUV on a regular basis; only one restraint system (car seat) was observed in the vehicle.
Child information forms are not on file for seven children currently enrolled, including the child present today.
Outdoor play equipment is not set up for children's use; the majority of the equipment is in disarray in two piles and stacked against the fence.
The program does not have a current equipment inventory on file. The last one was completed in April of 2022.
The director has been transporting children ages three and younger without another personnel member present in the vehicle to count in ratio; this includes one infant, a one-yr-old child, and a three-yr-old; today the one-yr-old child was transported along with older children; observed by licensing.
Two of the four handwashing sinks do not have handwashing procedures posted; the sink by diaper changing table, handwashing sink in kitchen.
One bathroom sink is missing the hot water knob; the hot water is not operable.
Personnel attendance records are not being maintained and were not available.
Immunization records are not on file for six children currently enrolled, including the child present today.
Children's attendance records have not been documented from 1/31/24 to present.
No parents have completed initial or current liability insurance forms or compliance file forms; the forms are lacking for all but four children and these were last completed in 2018.
