Complaint:
Facility - Mold is present inside the facility.
Plan to Correct
Upon notification of visible dark spotting identified as mold, the facility immediately secured and cleaned the affected area and conducted a preliminary internal inspection. A licensed mold assessment professional will be contacted to evaluate the area and determine the appropriate corrective action. Any impacted materials will be removed or professionally remediated as recommended. The source of moisture will be identified and repaired as necessary. HVAC filters will be replaced immediately, surrounding vents will be inspected and cleaned, and routine moisture and ventilation checks will be added to ongoing maintenance procedures to prevent recurrence.

340:110-3-304(a)(1) - The facility and vehicles are clean, sanitary, and free of clutter and litter, including paper accumulation. The facility and equipment have accumulations of no dust, dirt, soil, food particle, or other debris accumulation.
