At the time of licensing's visit, the two exit gates were locked with a chain wrapped in plastic.
At the time of licensing's visit, one child was sleeping on a cot blocking the exit.
Personnel job positions are not listed correctly on the Direct Care Organization, therefore the program is not maintaining all items listed on the Direct Care Facility Handbook.

Ratio - Staff child ratios are not being maintained at all times.
Plan to Correct
The director will maintain ratios and group sizes at all times. The director will make sure the rooms have enough teachers present when having to leave the program.
Regulation Description
340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.
Complaint:
Supervision - Children were napping in three different classrooms, with only one staff person present in the building.
Plan to Correct
The director will ensure that all children and classrooms are supervised at all times. The director will ensure to have enough staff on site when having to leave the program.
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
