Plan to Correct

The classroom was closed from 3/14 to 3/22. During that time meetings were held with parents to discuss behavior issues. Graduated days were implemented to all four students. Two will attend in the morning only and two will attend in the afternoon only. One employee is no longer employed at the center. Education Coordinator will be on-site for training and support until more structure and active supervision is observed. Active supervision training will be completed by the Director. She will also complete a training called "responding in the moment where behaviors challenge adults". DHS will provide requirements training at the pre-service training.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
