Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
Monthly drills have not been documented for January 2025. Drill sheet for 2024 is still posted.
One staff member has an expired PDL as of 1/7/25.
One staff member has not completed the required amount of training within the certification year- 4 hours of training has been completed and documented.
Compliance file does not include program's most recent visits.
Monthly drills-fire, smoke detector, carbon monoxide, and tornado- have not been documented for June or July. Last drill documented was 5/20/24 for fire and tornado.
On 8/8/22 and audit was conducted for the month of June 2022. During the audit, it was found that the provider did not maintain attendance records for children in care. Children who were swiped in at the facility were either missing from the provider's attendance records or there was no record of the children's attendance for that day.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
