There were no lesson plans in any classrooms.
There was no thermometer in the refrigerator where the breastmilk is stored.
There was sanitizer spray on the edge of the counter where the children use the bathroom.

Supervision: A child received an injury while outside due to lack of supervision.
Plan to Correct
Licensing received the following plan of correction: Staff will remain diligent making sure that all children are within sight. If a wagon is used staff will make sure that all children are properly restrained and will pull the wagon backwards.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
