Children who were swiped in at the facility were either missing from the program’s attendance records, or there was no record of the children’s attendance for that day/time. DHS policy 340:110-3-281.2(c)(5)(B) states daily attendance records are to be maintained on paper or electronically for at least 12 months.

  • No staff in room with 3 and older. The ratio was 13 to 0. There were two 3 Year Olds; Another class was understaffed. There was 10 to 1 ratio in the Infant's, 1's, & 2's classroom.
  • No physical environment completed. The last physical environment checklist was dated 3/24/21.
  • Central detection system not tested and maintained. The system was test last on 3/23/21.
  • One staff has an incomplete criminal history.
  • No staff in the 3 and older classroom supervising children. There were 13 children napping in the room.
  • Mop bucket with 3 inches of water was left unattended in the hall accessible to a 3 and older classroom of children.
