Three staff members who are no longer employed remain associated to program's DCO account.
Two staff employed since 12/2022 have not been verified as being employed on the DCO account; one person was verified as holding a Probationary Master Teacher position since 08/2020.
Two disqualified probationary master teachers have positions remaining as PMT's for the program; one PMT not listed as holding that position on DCO account.
Water temperature in classroom restrooms are registering at 77 degrees Fahrenheit.
Last week's lesson plans were still posted in infant and 3-year-old classrooms.
One teaching staff member has an expired PDL certificate as of July and renewed this month.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Pending. Due by 08/15/2024. Received on 09/12/2024. "Staff member will be in doorway to supervise at all times. Ensure all staff members are aware of bathroom protocol. Separated the siblings so that they are no longer in the same room. Bathroom breaks done with no other children from these siblings."
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
