Labeled diaper rash cream and Orajel in the infant room did not have a signed permission form on file.
The following staff do not have a current PDL and have been employed more than 12 months: SJ, EJ.
This non-compliance was not observed at the visit. It was documented on an OIG compliance audit report for a visit that occurred on 3/29/23. It was determined during the audit the provider did not maintain adequate attendance records, including arrival and departure times for all children in care.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

1) Additional Non-Compliance Found During Investigation: Discipline and behavior guidance - Staff member seen in video handling infant inappropriately without regard to physical injury.

2) Additional Non-Compliance Found During Investigation: Learning program principles - Infant was observed on video in stationary activity center longer than 20 minutes.

Regulation Description
340:110-3-289(j)(3) - do not remain for more than 20 minute increments in equipment restricting freedom of movement, such as rest equipment, swings, high chairs, or stationary activity centers, except while eating in high chairs or similar stationary equipment. Children only sleep in appropriate rest equipment, per OAC 340:110-3-296.

Regulation Description
340:110-3-280(b)(1) - Abuse and neglect. Any person who has reason to believe a child was abused or neglected, per Section 1-1-105 of Title 10A of the Oklahoma Statutes (10A O.S. § 1-1-105) is required to report the matter promptly to the OKDHS Child Abuse and Neglect Hotline at 1-800-522-3511, per O.S. § 1-2-101. Failure to report is a misdemeanor offense and upon conviction is punishable by law. Failure to report with prolonged knowledge, six months or more, of ongoing abuse or neglect is a felony offense.

1.) Prohibited Individuals: Serious - Program allowed an individual with a known criminal history restriction to work on the premises of the childcare facility during hours of operation.
Plan to Correct
Our policy is to have everyone fingerprinted, regardless of the area of the building or program they work in. If there is an issue with their fingerprints, they will be escorted off the premises immediately without the opportunity to re-enter the building or any of our programs. The only exception to this policy is if it is decided to submit a waiver request to OKDHS. The individual will still be prohibited from being in the facility until an OKDHS waiver is approved.

1.) Transportation - Transportation permission not obtained prior to transporting a child.
Plan to Correct
When a child enrolls in the center and transportation is requested, a Transportation Request Form (see attached) is completed by the individual completing enrollment paperwork (i.e.
parent/guardian).
The parent is informed that the request will be submitted to the Transportation Director and
they will let them (the parent) know if the transportation request has been Approved or
Denied. If the approved, a discussion between the Transportation Director and the parent
regarding the particulars of the transportation takes place.
The Transportation Director then adds the child to the Route List (see attached) for the driver and gives the driver a copy of the Route List and a copy of the Transportation Request Form.
A child will only be transported once a Transportation Request form is approved with a copy of the request given to the driver AND their name is added to the route list.

340:110-3-305(c)(1) - Transportation and field trip permission is maintained, per OAC 340:110-3- 281.4(b).
