Allegation Findings
Substantiated
Complaint:
Serious: Supervision--child left unattended
Plan to Correct
Although we disagree with the findings in relation to the complaint, we will make adjustments to our morning routine to ensure that no staff leaves a classroom that has a child for any amount of time. Alyssa will keep a door clicker on her person while in a classroom to ensure that neither her or the other morning teacher has to leave their classroom to open the door for a parent. We will continue this practice until such time that childcare centers are funded appropriately so that an extra person can be in the office at all times to monitor and manage the front door.

Allegation Findings
Substantiated
Complaint:
Serious: Supervision--Not being maintained on and off premises.
Plan to Correct
The gate and the lock have both been replaced and fixed in order to maintain security within the playground and allow for the gate to open by an adult from the inside. This was fixed completely by November 7th, 2025. The two teachers involved in the incident have been separated and are now in two separate rooms. Both teachers will be overseen by another staff member in their classrooms, and have completed Supervision and Playground training, along with our other staff members. The teachers involved have been given formal write-ups regarding supervision. Both teachers have watched the video and we have discussed where the errors occurred and what could have been done to prevent the serious situation. In addition to fixing the gate and completing training for supervision and playground safety, our program will maintain "name to face" checks using ProCARE, a member of management will monitor classes while they are on the playground, daily playground safety checks will be more thoroughly completed by management daily, and staff will walk the playground prior to taking their class outside to ensure no hazards have been missed. In the event that active supervision is not being provided by a staff member, they will be terminated. Our center will continue to make changes and adjustments as needed to ensure that proper supervision is maintained throughout the building.
