Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
Licensing was informed on 12/1/23 the following: We are awaiting more results for the lead, they will send to me once they receive and I will get a copy to you at that time. Licensing called head office and asked about the lead results on 12/20/24 and was informed, It is a waiting game. I will have to contact my person and see if they have them. If not I will send you the most current results or you might already have them.
Licensing looked up the Schools water testing on http://sdwis.deq.state.ok.us/. The Schools lead testing has not been done since 12/10/21. The School is only required to get lead every 3 years and the last nitrate test was on 5/12/24.
Licensing has asked for water testing results. Licensing was only provided water testing results for bacteria and nitrates. Licensing has not received lead test results from program.
Licensing observed no smoke alarms or carbon monoxide alarms in the cafeteria. Director stated program uses schools central detection system for smoke and carbon monoxide. Licensing could not verify if the central detection system has been inspected and tagged with in the last 12 months.
Licensing observed that program information and emergency numbers were not posted at the program, that are to include: Program information- name, address, with main cross streets or directions to the facility and Emergency numbers-911 where available, or local law enforcement, fire department, ambulance service and poison control number.
Licensing observed two (2) personnel on site and one (1) individual listed as a subsitute that personnel information sheet was not submitted to licensing for the program and personnel was not associated with program in CCMASS.
Licensing observed program is on well water. Program has not had water tested by DEQ.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
