Three wires extending from the electric pole to the side of the building are approximately 5-6 feet off the ground in the outdoor play area & accessible to children.
Vehicle used for transporting children has an expired tag as of 12/2024.
Program does not have a fully qualified master teacher employed at this time.
Probationary master teachers are not listed under the program's direct care organization account as holding the job title; substitute teacher who is no longer employed remains associated to the account; one teacher who is a substitute since previous monitoring visit is still listed as a teacher under the account; two owners listed although only one owner appears on the Proof of Ownership that was submitted to Licensing.
One assistant teacher and one teacher employed consecutive since 2011 and dropped to substitute status effective May have expired PDL certificates and have not submitted a renewal application.
