Plan to Correct

Plan of correction completed on 8/26/26 then corrected and resigned on 9/2/26. The fence and gate were repaired on 8/17/26. Owner/ Designee Sena Yesslith had a meeting with staff on August 18th to address the incident and Supervision. Owner/ Designee also talked with interim Director Addie McGill about utilizing inside gym for play during extreme weather conditions. Ms. Yesslith will also coordinate with licensing for a licensing requirements training and for active supervision training for staff. She will consult with Interim Director and Leslie D. Smith who is the Director of Staff to ensure they are actively providing oversight of staff to ensure supervision is maintained at all times.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
