All 1's classroom with portable sink did not have hot water.
Butt Paste brand diaper rash cream in all 2's classroom was not labeled with child's name.
Diaper rash cream (Butt Paste brand) was accessible in unlocked cabinet under the diaper changing table in the all 2's classroom (7:1 ratio).
Portable sink present in all 1's classroom (5:1 ratio) used for handwashing without a permanent plumbed sink present.
Both sinks in the school age building do not have operating hot water, sink in the all 1's (5:1 ratio) and all 2's (7:1 ratio) today located in the main building does not have hot water.
Lunch today was missing another fruit or vegetable.
Lesson plans were not posted in 2 classrooms (2 & older w/ 7:1 ratio and all 1 with 6:1 ratio)
Multiple containers of diaper rash cream was accessible a classroom (1's classroom) in the cabinet under the diaper changing table.
Four teacher used in staff/child ratios have expired PDL certificates and one teacher did not obtain a PDL certificate within 12 months of being used to meet staff/child ratios.
One teacher present today did not submit an online Criminal History Review Request to OBI; one teacher has closed/invalid preliminary results.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
Background investigations have been conducted for all new employees. In late August, upon return to the facility as Owner/Director, the previous director was removed. The owner/director immediately reviewed all employee files to ensure BI's had been performed and completed IAC with OAC 340:110-3-282(a)(1)(C).

The facility has an updated weekly menu that includes balanced meals for breakfast, lunch & snacks. The facility owner/director oversees the weekly grocery shopping and double checks to make sure each meal is fully balanced with fresh and nutritious foods daily. Upon return of the facility owner/director, the previous director was removed from the position. The facility owner/director immediately reviewed and revised the daily menu to ensure all emails and snacks are balanced and provided with all applicable guidelines.

340:110-3-298(b)(1) - Meals and snacks. Meals and snacks are required to meet the current Child and Adult Care Food Program (CACFP) guidelines, per Oklahoma State Department of Education (OSDE), with the exception of infants, per (f) of this Section.
