Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
Licensing observed two personnel to have been employed over 12 months to not have maintained a current PDL within 12 months of expiration.
Licensing observed director not having obtained at least the required number of professional development clock-hours to maintain a current Oklahoma Director's Credential on site at the time of visit.
Licensing observed program to not have conducted a vehicle quarterly inspection. Last vehicle quarterly inspection was documented on 5/28/24.

Licensing observed the program to not have completed a new physical environment checklist for 2024. Last physical environment checklist completed on 7/1/23.
