Based on this child's allergy, we have removed all products containing pears or pear juice from the center's menu. We no longer serve pears or any fruit containing pears or pear juice to any classroom. The classroom allergy lists are updated monthly and any time we have a new child with an allergy. It has also been noted by the parent that this child has eczema. We did not have any evidence that this child ate pears. This could have been an eczema outbreak, now notified us to the parent after that incident.

SERIOUS: Supervision - Not being maintained.
Plan to Correct
In this situation, two children pushed open the doors to the outside while the teacher was sitting with the group of children who were eating breakfast. The teacher saw the children exit the door and she immediately tried to stop them but couldn't reach them in time. They ran out to the playground but at no time were they out of her sight. The teacher immediately called the front desk while she was watching the children outside and the assistant director went out and retrieved the children within a matter of a few short minutes. At no time were the children out of someone's sight. We believe the teacher acted appropriately for this incident as she was trained to do. We self reported this incident. We added door alarms to every classroom door on the same day this incident occurred so that teachers are able to immediately hear any doors opening by an alarm sounding and are able to react quickly. We also conducted additional staff trainings in supervision at our 8/29 staff development day to ensure every staff member is aware of the correct procedure if this were to happen again.

As soon as we were notified of the allegations, the employee was placed on suspension. The employee has not returned to employment at the center at any point in time. As of 9/2 the suspension ended and the employee is no longer employed with us. All staff were retrained on appropriate child guidance in one to one meetings as well as in our staff development day on 8/29. In order to better meet the children's needs and to enhance classroom management, we reduced the school age ratio to 1:15 for all ages. Center leadership also increased presence in the school age classrooms conducting regular teacher and classroom observations to ensure the teachers were engaging appropriately with all children and supported teachers when needed for behavior challenges.

340:110-3-280(b)(1) - Abuse and neglect. Any person who has reason to believe a child was abused or neglected, per Section 1-1-105 of Title 10A of the Oklahoma Statutes (10A O.S. § 1-1-105) is required to report the matter promptly to the OKDHS Child Abuse and Neglect Hotline at 1-800-522-3511, per O.S. § 1-2-101. Failure to report is a misdemeanor offense and upon conviction is punishable by law. Failure to report with prolonged knowledge, six months or more, of ongoing abuse or neglect is a felony offense.

The training consisted of the following topics: Safety and Supervision: Child Supervision Records/Head Count and Transition Procedures. Staff were all re-trained on all supervision practices including maintaining proper ratios and visual supervision at all times. Transition training included practices that engage children and ensure Teachers are able to supervise children throughout the transition from one classroom or area to another while maintaining ratios at all times. In this isolated incident, a teacher allegedly walked out of the classroom with a non-employee observer, who was not background cleared. Teachers have been trained that any new staff member who is not yet completely cleared may not be left alone until a center leadership member informs them otherwise. This includes any visitor or observer. No one without clearance will ever be left alone with children.
