Plan to Correct

We use LineLeader for child daily logs, but as a backup and to make sure parents are getting accurate daily reports we have reimplemented paper daily logs for parents to receive at the end of the day at pick up.

340:110-3-281.4(b)(2) - Infant and 1-year-old daily record. Daily records for 1-year-olds and younger are available in the child's classroom and include prompt documentation of eating, diapering, and sleeping activities.

Plan to Correct

All school age teachers have been retrained on ratios and supervision. We understand that the gym and classroom are two separate rooms and will be treated as such. New name to face forms are being used and checked every 30 minutes.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Plan to Correct

"We will continue doing daily sheets that show diaper changes that are for parents as well as our daily diaper logs that we keep for our records. Admin will check throughout the day to make sure proper diaper changes are being done."

Per the Notice to Comply dated 10-13-22, Parents are and will continue to be notified of any incidents of known minor injuries.
