One teaching personnel, employed over 90 days, does not have documentation of current ELCCT.
One teaching personnel does not have documentation of current CPR and first aid certification.
One employee’s OPDL had expired, and when licensing checked to see when it was submitted, it was found to have been submitted after the expiration date.
Licensing observed, there was a 13:1 ratio in the 2-year-old and older classroom.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

1. Discipline and Behavior guidance: Inappropriate Discipline-Teacher grabbed child's arm and left marks.
Plan to Correct
Teacher was terminated on 04/14/2025, when program was made aware of incident. Program contacted Child Care Services and the parent at that time. Will have a staff meeting on 05/09/2025, to discussed discipline and remind staff that they are not to use physical discipline at any time. Will discuss with all staff that if they have to move a child, that the staff will use the child's hand. If child pulls back, that the staff will let go of the child and ask for help from another teacher.

Plan to Correct
The teachers will be more alert with the children and watch them better. They will keep a better eye on them and not be distracted. And they will have to do better on reports for parents.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

3. Supervision: non-job-related activities: Teacher in classroom counted in staff child ratio, had a visitor on a regular basis and were on cell phone.
Plan to Correct
We will have a meeting about visitors and phones. All phones will be put away or kept in cars. If a visitor comes in, another teacher will stop in the classroom for them to step outside. The visitor will remain in the office or outside.

Plan to Correct
Will talk with staff about pulling the child by the arm and how to properly lift the children. Will have staff do a behavior and guidance training either on CECPD or with Resource and Referral. Will also schedule a Licensing Requirements training with Child Care Services.

Additional Non-Compliance Found During Investigation: SERIOUS: Ratios and group size: Classroom was out of ratio, when program personnel, went to the kitchen leaving 3 children alone in the classroom.

We will make sure classrooms are covered. If you need to go out of room, you will make sure you have a teacher to relieve you.

340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes.
