Two staff members who have been employed more than 12 months have an expired PDL.
Two staff members employed more than 12 months did not complete the required number of professional development hours for their PDL renewal.
Mice droppings found throughout the kitchen on surfaces and on different food items such as cans, boxes with baby food containers, and box with mayo containers.
The program had a health inspection on 12/11/24, and mice droppings were found. CCLS observed mouse droppings in the kitchen area of the program.
Plastic food utensils are stored in a box with an open lid under a food prep table in the kitchen.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Supervision: Program staff are not supervising the children in their care.
Plan to Correct
The center has hired additional staff so there will be 3 staff assigned to each classroom to always maintain staff/child ratios. When a position is vacated, the center will actively seek to fill positions promptly.
Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
Complaint:
Staff/Child Ratios: Program was out of staff-child ratio due to Supervision concerns.
Plan to Correct
Each staff member will be assigned "focus" children. This is so they can focus on observations, developmental assessment, and all those things in their focus group of children. To always maintain child counts the classroom staff will be required to fill out a daily child count worksheet for their focus children and they will be responsible for maintaining child counts during daily transitions.
Regulation Description
340:110-3-286(a) - Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

Additional Non-Compliance Found During Investigation: Program did not notify parent of incidents in a timely manner per licensing requirements.
Plan to Correct
When there is an incident involving a child, staff will immediately fill out an incident report. Any serious incidents, including but not limited to, shoulder up incidents or a child left unsupervised, will be reported to the Director immediately and an incident report will promptly be given directly to the Director. The Director will verify that the incident took place via video observation
