Diaper changing area in infants, ones & two's classroom had children's cups sitting on diaper changing area.
Licensing observed a teacher leave 11 children (3 year olds) unattended in the classroom while she went to assist a child in the restroom that was unsupervised.
Licensing observed 11-3 year olds alone in class room while the teacher stepped out to help a child in the restroom across the hall.

Complaint:
Nutrition - Due to bottle and breast milk not labeled with child's name wrong bottle was used and a child was fed incorrect breast milk.
Plan to Correct
This was an isolated incident that was handled internally. Program will ensure that all bottles and students belongings are properly labeled before student is permitted to staying at facility.
