Two classrooms (Farm and Insects) with children under age 6, had outlets that were uncovered and accessible to children. The Farm classroom cover was located on a nearby shelf. The Insect classroom did not have easily accessible covers in the classroom.
First aid kit located in the kitchen was missing non-medicated adhesive strips, disposable non-porous latex gloves, and a copy of the program information and emergency numbers posting.
Last enrolled child does not have a copy of the immunization record or exemption on-site. Child started 3/19/2025.
Four teachers do not have proof of orientation completion.
One teacher does not have a current PDL and does not have the completed hours for the PDL.
The physical Environment Checklist was last completed on 11/3/2023.
Five teachers do not have orientation training as required.
One teacher does not have ELCCT and they were hired 12/18/2023. This is the second documentation of this non-compliance.
The area in front of the bottom of the slide on the older playground had grass growing in the use zone.
There was a broken & loose tile in the boy's restroom upstairs. There was carpet that was not firmly attached to the downstairs stage on the stairs. Two holes was forming in walls on either side of the stage where sheetrock was visible and accessible to the children.
One staff does not have a current PDL since 1/30/2024.
Two teaching staff do not have proof of completed ELCCT on-site.
One teacher has 11 hours reported to have been submitted for their PDL in the past 12 months. 1 more hour required for a current PDL.

Classroom with 1 year old and older children had a sink that the hot water was not working. The water was dripping out and not usable. Cold water was running as normal.
One classroom upstairs had peeling paint on one wall accessible to children.
In the eating area for the 1YO children there was Eleven children with five 1 year olds. There was only one teacher. The additional teacher was inside another classroom putting down sleep equipment.
A one year child was served a whole chicken nugget.
Six staff were missing proof of completion or had not completed Orientation training.
Two teachers were missing proof of completion or did not complete the ELCCT.
Two teachers did not have proof or completion or did not complete the CPR and First Aid certification.
Program has not been completing the form every 12 months. One form viewed last completed in 2021.
Infant lesson plan was not dated and was from Fall time. Lesson plan also listed generic activities such as standing, finger paint, Free play, and teacher choice.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.

Plan to Correct
Teacher removed known allergy from child once realized during the meal. The Director will create a memo to train the person making meals and teachers serving the meals in the classroom to review the allergy posting prior to making and serving meals to children. All personnel will be made to sign this and a copy will be sent to child care licensing. Director will add this training to the onboarding process as well.

Supervision- SERIOUS- "Children gained multiple injuries while at the program and teachers do not know how they got the injuries."
Plan to Correct
Staff was given a rundown on the importance of maintaining constant supervision to children in their care. They have signed a paper acknowledging the talk. We are hoping to receive a training in June from CCS to address supervision and behavior and guidance. POC date due 5/14/2024.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Medication-SERIOUS-"Medication was put a child's injury without a completed medication form."
Plan to Correct
Parents have filled out a diaper cream permission form. Staff have been made aware of the serious non-compliance and we will impose regulations on staff that they must clear any administering of any medicine to the administration. POC date due by 5/14/2024.

340:110-3-295(c) - Parent permission. Parent permission is obtained prior to administering the medication. Documentation is maintained per OAC 340:110-3-281.4(b).

We will continue to use "boo boo" reports and have immediately begun using the correct Injury and Poison Exposure Report. We have been doing this since 3/15/2024. Date Due by 5/14/2024.
