Behavior & Guidance/Discipline (Serious): Weighted vest used on child.
Plan to Correct
Teacher dismissed immediately, all weighted material have been removed from the classrooms, teachers received training on redirecting distracted behaviors, Learning Tree has updated the teacher Policy & Procedures Handbook to include appropriated practices and require signatures acknowledge of said policies, and cameras are being installed in all classrooms for monitoring children and teacher interactions.

340:110-3-288(c)(1)(F) - restraining or tying to restrict a child's movement by any means other than holding. Holding is used only as long as necessary for the child to regain self-control.

Additional Non-Compliance Found During Investigation: Hazard: A pill (medication) was found on the playground.
Plan to Correct
Playground is inspected for hazards by teacher prior to all children being allowed on the playground and cameras are being installed throughout the playground area. Footage daily will be reviewed by both Learning Tree and church daily, due to the fact the playground is open to the public.
