Plan to Correct

Program staff will call parents when child is injured and will be told that an incident report is waiting for them at pick up. If a child is hurt and there is not a mark, an incident report will be completed and parents will be contacted.

Program staff will have whom ever picks up child sign an incident report and a copy will be sent home with that person. The program will keep the original on file as well.

340:110-3-281.4(b)(9) - Injury or poison exposure report. An OKDHS or program form containing similar information is completed for each injury or poison exposure occurring while in care.

Plan to Correct

Licensing received the following plan of correction on 12/20/22: We will have two staff in the classroom at all times. A third staff will be in there from 9:15 to 3:15 pm. One staff will be shadowing child to ensure the safety of the staff and children. We will continue to monitor the child.
