  • Direct care organization needs to be updated. Director is listed as assistant director and owner is still listed as the director.
  • Staff employed on 5/31/23 FA/CPR expired on 6/30/25 and does not have current training at this time.

  • Worker observed three staff to not have current PDL certificates. 1 staff employed on 11/21/21 PDL expired on 1/19/23. Last submitted 1/18/22 1 staff employed on 7/21/21 PDL not awarded. PDL has not been submitted. 1 staff employed on 7/10/21 PDL not awarded. PDL has not been submitted.
  • Worker observed areas of the fence to not be secure. T post leaning and part of the fence leaning due to tree branches falling on fence.
  • Worker observed peeling paint on the walls in the building and the material on the corner of the wall to be coming off. Worker observed exposed wall in several areas of the building.
