Program could not locate current physical environment checklist or current equipment inventory.
Outside sand table with exposed rusted nails accessible to children.

Plan to Correct
Plan of correction provided by program: After receiving that this was substantiated, had a meeting with all employees. Reviewed that all employees with discipline at the daycare. Staff meeting: Reviewed the complaint that was substantiated for program personnel screaming at children. Just reminded everyone that there is no screaming at children.

340:110-3-288(c) - Inappropriate discipline. Personnel are prohibited from using or threatening to use inappropriate discipline.

Parent Communication and Family Engagement: Parents are not given access to program and wait on porch to drop off and pick up children.
Plan to Correct
Program provided the following plan of correction: We sent a note to all parents, guardians, and grandparents dated 11/11/24. Since 11/11/24, once the door is answered and verified, parents are allowed inside the daycare at drop off and pick-up.

340:110-3-293(c) - Access. Parents of enrolled children are allowed reasonable access to facility areas used by children.
