The teacher for this complaint is no longer with the facility since concern was brought to our attention. All staff are trained on positive redirection as well as proper nap time procedures during onboarding and this was not within line to our company standards in with how this teacher chose to act. All current teachers have been retrained as of 7/21/25.

340:110-3-296(e)(3) - Restraints. Restraining children on rest equipment is prohibited.

340:110-3-296(e)(4) - After resting. Once children have rested, they are not forced to remain on the rest equipment and are provided quiet activities until rest time is over.

Notice to comply received 1/27/25 from director Felicia Cox - All staff had a retraining on supervision policies including use of electronic devices were held accountable per our policy for this violation. Any moving forward will be as well as we strive to meet all DHS and LCG policies.

340:110-3-287(c) - Non-job related. do not participate in personal activities that may interfere with supervision, such as visitors, phone calls, or electronic device use.

Plan to Correct

Notice to comply obtained during the 10/2/23 partial visit - The two staff in charge of supervising the classroom the child was in were terminated immediately. The program is requiring all staff to complete supervision training and will be contacting a child care coach for more training options for staff. The programs director will be taking off the cabinet doors in all classrooms and will walk the halls more often checking in with teachers and supervising the classrooms on the cameras in her office. The program will submit training documentation once staff have completed supervision training to licensing and ensure all staff have a refresher as needed and all new staff will take the supervision training the company offers before entering the classroom to supervise children.

Regulation Description

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.
