Licensing observed a crack on a plastic seat on metal play equipment, causing a pinch hazard.
Licensing observed the fire extinguishers to not have been serviced within the last 12 months.
Three bottles in the infant room refrigerator were not labeled with names.
An infant was drinking a bottle in the swing. The swing was in motion. The infant displayed the ability to hold the bottle without assistance or needing to be held while drinking a bottle.
Two infants were asleep in two separate swings. Swings were in motion.

Plastic climber and crawl through apparatus in outdoor play area is in disrepair; large crack on the side of blue climber and large crack in the landing area of the climber.

Notice to Comply - Staff member in question was terminated as of 12/08/23; appropriate discipline expectations will be reviewed with all staff; Conscious Discipline training is scheduled January 2, 2024 for all staff to attend. Discipline methods used by staff will be closely monitored by leadership.
