  • 1 trike has a broken seat; 1 plastic trike has quarter-sized hole in handle bar; 1 yellow scooter has missing platform.
  • Paper towels were not available in school age classroom
  • Lesson plans for this week were not posted in school age, 4 year old and older, and both 2 year old and older classrooms.
  • One teacher employed longer than 12 months has an expired PDL certificate as of February.
  • Fire extinguishers expired 2/28/25 and have not been retagged.
  • Impact material under large outdoor play structure slide is about 1 inch depth.
  • One teacher was verified as a Master Teacher and does not qualify, director position listed twice.
  • SERIOUS: Self-reported incident occurred on 3/14/25 involving a school age child sleeping inside classroom for 10-15 minutes while the rest of the classroom was outside at recess.
