Immediate actions taken: Increased Staffing: Additional staff members have been scheduled during peak activity times to ensure appropriate supervision ratios are maintained. Classroom area: Room arrangements have been re-evaluated to ensure that all areas where children are active are monitored effectively. Training Session: A mandatory training session on supervision standards has been conducted for all staff in this facility to reinforce the importance of supervision and intervention protocols. Long-Term Corrective Measures: Staff Training and Development: Onboarding Process: Revise the onboarding process for new staff to include comprehensive training on supervision expectations and the importance of maintaining constant awareness of children's activities. Supervision Policies and Procedures: Review Active Supervision of Children Policy: Develop and distribute a plan that outlines specific responsibilities within the policy for staff members regarding supervision and intervention and ensure all staff receive copies. Supervision Checklists: Implement daily supervision checklists for staff to complete during their shifts, documenting areas monitored and any concerns noted. Monitoring and Evaluation: Regular Observations: Conduct regular observations and evaluations of staff during activities to ensure compliance with supervision standards. Feedback will be provide to staff based on observations. Incident Reporting: Immediately report incidents of a lapse in supervision to DHS and investigate the process to address any weaknesses. Communication with Parents and Stakeholders: Parent Meetings: Include active supervision policies in Parent Meetings to ensure cohesive communication and feedback to formulate and update policies and procedures as needed. Regulation Description
340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Immediate action take: Contacted DHS for further explanation. Requested review, appeal. Parents contacted as required (b) Parent reporting. Parents and personnel communicate daily and when additional required, per OAC 340: 110-3-280(d). Plan of action: Report all incidents regardless of level of concern, even with parent contact. Active supervision training was implemented and completed 10/17/2024. Monitoring and Evaluation Regular Observations: Conduct regular observation and evaluations of staff during activities to ensure compliance with supervision standards. Feedback will be provided to staff based on observations. Incident Reporting: Immediately report incidents of a lapse in supervision to DHS and investigate the process to address any weaknesses. Timeline for Implementation: Immediate Actions: Completed Immediately. Monitoring Plan: Implementation of the regular observation and evaluation plan to begin by 10/21/2024. Conclusion: The incident occurring at the Kingston- Inca Head Start was not reported by teh next day due to the miss-understanding of the DHS regulations on the required report of events, DHS defines "imminent risk" as: Exposure to imminent risk of harm means "Harm or threatened harm to the health or safety of a child" means any real or threatened physical, mental, or emotional injury or damage to the body or mind that is not accidental including, but not limited to, sexual abuse, sexual exploitation, neglect, or dependency, per 10A O.S. 1-1-105 2&7. The child was never in any danger of harm of threat to his health or safety. The child in question was in a hiding spot and was laughing while Ms. Price searched for him. There was never any physical, mental or emotional injury or damage to his body or mind. This incident was not intentional, but occurred as a result of behavior known to parents as being problematic. In effort to comply with DHS regulations and reporting, the parents were contacted within 30 minutes of the incident and the parents have been invited to volunteer at the center. Parents of enrolled children are allowed reasonable access to the facility areas used by children. Moving forward DHS will be notified of accidents and incidents to ensure compliance with all DHS regulations.
