During the OIG visit conducted on 04/03/2025 it was determined the provider did not maintain adequate attendance records, including arrival and departure times, for all children in care.
Self-report- Child left unattended in vehicle due to failure of personnel to follow a two step method of inspecting the vehicle before leaving the vehicle.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. Plan of Correction · The driver that left the child in the vehicle is no longer driving children. · Policies have been reviewed and updated with all drivers. · Drivers are to conduct a walk through after each trip. (previous policy) · Drivers are to complete the no child left behind procedure after each walk through: o New bus- has an electronic system o Old bus- has a picture system. The driver is to move the picture from the front to the back of the bus after completing the walk through. Before the bus leaves for the next trip, the picture is brought to the front of the bus. o Van- has a picture system. The driver is to move the picture from the front to the back of the van after completing the walk through. Before the van leaves for the next trip, the picture is brought to the front of the bus. o We have ordered the no child left behind system for the old bus and the van. Once we receive them, the picture system will no longer be utilized. · The drivers and front office staff complete a name to face check-in and out system. · Doorbells have been installed on all three vehicles, so that an alarm will sound if someone is left on a vehicle. · All drivers have completed the following course: transportation and field trip safety for child care centers. · We have continued to utilize our Transportation Roster to document when children get on and off the bus/van and the Chickasaw Nation bus. o Schools: Will Rogers, Charles Evans, and Lincoln · The school age teachers call the front desk to report any children that are absent.

All classroom staff (teachers, assistant teachers, and floaters) are required to take Foundations of Positive Guidance, Classroom Management, Building and Physical Premises Safety: Indoor Environments and Active Supervision.
Staff- Active Supervision has been discussed with the teachers and they will be completing an active supervision course. We will follow the guidelines of active supervision and always have the safety of the children at the forefront of our actions.
Curriculum Coordinator and Afternoon Supervisor-The curriculum coordinator and afternoon supervisor will physically check-in with the teachers throughout the day at unannounced times on an hourly basis. this will allow the administration to be proactive instead of reactive and it will allow the administration to ensure that the staff is always providing active supervision.
Premises/Building-It has been found that the doors don't lock from the inside, however we are having a fence installed that will include both classrooms that have the exit doors facing "G" St. The fence will have a gate that will include a child proof lock, so that the sidewalk can still be utilized as a fire escape route.

340:110-3-287(a) - Supervision. Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Regulation Description
340:110-3-280(b)(1) - Abuse and neglect. Any person who has reason to believe a child was abused or neglected, per Section 1-1-105 of Title 10A of the Oklahoma Statutes (10A O.S. § 1-1-105) is required to report the matter promptly to the OKDHS Child Abuse and Neglect Hotline at 1-800-522-3511, per O.S. § 1-2-101. Failure to report is a misdemeanor offense and upon conviction is punishable by law. Failure to report with prolonged knowledge, six months or more, of ongoing abuse or neglect is a felony offense.
