Licensing observed that five personnel with annual infection control training that has not been completed within the last 12 months.
Current Director does not have an Oklahoma Director's Credential.
Documented on Health Inspection Conducted on 06/25/2025 - Milk was being held in a reach in cooler that was between 50-58 degrees. Program discarded milk at the time of the inspection.
During the OIG visit conducted on 04/03/2025 it was determined the provider did not maintain adequate attendance records, including arrival and departure times, for all children in care.
Self-report- Child left unattended in vehicle due to failure of personnel to follow a two step method of inspecting the vehicle before leaving the vehicle.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. Playground Procedures Policy
To maintain a safe and consistent environment for all children and staff, the following procedures must be followed at all times:
1. Playground Access
- No class is permitted to use their classroom door to enter or exit the playground.
- All classes must use the designated hallway exit and entry points assigned for playground transitions.
2. Name-to-Face Verification
• Classroom teachers will complete a name-to-face check before leaving the classroom to go outside.
• Classroom teachers will complete a name-to-face check before leaving the playground to come back inside.
• An administrator will verify each name-to-face check before the class goes outside and again before the class comes inside.
• Classroom teachers will only complete each name-to-face check once—once before leaving the classroom and once before leaving the playground.
3. Communication Equipment
- Walkie-talkies are required whenever a class is on the playground.
- Walkie-talkies will be stored on the metal shelf outside the laundry room.
- Staff must ensure a walkie-talkie is taken outside and remains functional during playground use.
4. Staff Responsibility
- Classroom staff are responsible for ensuring all procedures are followed.
- Any concerns or safety issues must be reported to administration immediately.

Playground Fencing
Currently the children will enter and exit the front playground through the Yellow 1 classroom, and once the fencing is extended, they will utilize the front exit. This will allow the staff to walk out of the front exit door and enter into the fenced in walkway that was previously just an open sidewalk.

The driver is to move the picture from the front to the back of the bus after completing the walk through. Before the bus leaves for the next trip, the picture is brought to the front of the bus. o Van- has a picture system. The driver is to move the picture from the front to the back of the van after completing the walk through. Before the van leaves for the next trip, the picture is brought to the front of the bus. o We have ordered the no child left behind system for the old bus and the van. All classroom staff (teachers, assistant teachers, and floaters) are required to take Foundations of Positive Guidance, Classroom Management, Building and Physical Premises Safety: Indoor Environments and Active Supervision.
Staff- Active Supervision has been discussed with the teachers and they will be completing an active supervision course. We will follow the guidelines of active supervision and always have the safety of the children at the forefront of our actions.
Curriculum Coordinator and Afternoon Supervisor-The curriculum coordinator and afternoon supervisor will physically check-in with the teachers throughout the day at unannounced times on an hourly basis. this will allow the administration to be proactive instead of reactive and it will allow the administration to ensure that the staff is always providing active supervision.
Premises/Building-It has been found that the doors don't lock from the inside, however we are having a fence installed that will include both classrooms that have the exit doors facing "G" St. The fence will have a gate that will include a child proof lock, so that the sidewalk can still be utilized as a fire escape route.
