Two children were missing insurance notification forms
Two children in care did not have immunization records on file.
Assistant caregiver did not have record of current CPR/FA. Assistant caregiver was able to show CPR training on school transcript but cannot verify the source of the training and does not have first aid training.
Assistant caregiver caring for infants while the PCG was not present at the home does not have safe sleep training.
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
