Licensing observed no staff working for program have personnel forms submitted to licensing.
Licensing observed children's records to be incomplete during time of visit.
Licensing observed two staff to not have PDL's at time of visit.
Licensing observed assistant caregiver to not have any clock hours of professional development at time of visit.
Licensing observed the primary caregiver to not have DHS approved safe sleep training while caring for infants.
Program did not have personnel paperwork on file for other caregivers at the time of visit.
Licensing observed two dogs running freely in the children play area while some children were napping on cots and other children were sitting at table eating their snack

Notice to Comply received 11/15/24 - Provider will ensure fingerprinting and background check requests are completed prior to individuals providing transportation.
Regulation Description
340:110-3-85(a)(6) - Criminal history investigation verification. The family child care owner or primary caregiver is responsible for submitting to OKDHS Office of Background Investigations (OBI), criminal history investigations and obtaining dispositions on any charges shown on the report lacking dispositions.
