No Insurance Notification form completed for all enrolled children. One child with no Compliance File Notification form.
Two children over twelve months observed asleep on the couch.
Muscogee Nation Licensing workers observed on 5/1/24 one caregiver in a bedroom/classroom on the couch asleep with a child in his arms. There was 6 other children in the same room with caregiver one of the children was awake playing on their mat.
Muscogee Nation observed on 5/1/24 food uncovered and old items in refrigerator needed to be discarded.
Muscogee Nation licensing Worker was unable to locate thermometer in refigerator at the time of the visit on 5/1/24.

We will advise staff to report incidents
immediately as they occur and ensure
they are more cautious when managing
groups of different ages. Failure to report
incidents promptly will result in
immediate termination.
