Licensing observed that Primary Caregivers PDL level expired on 9/28/2025, Was requested 7/30/25, but provider states she received notification that qualifications were not met.
Licensing observed that provider only has 9 current hours of training.
Licensing observed that hand towel or paper towels were not available for children.
Licensing observed that two children are missing immunization or exemption forms.
CCS designee reported primary caregiver observed to be vaping twice on 10/14/2025 with children in care.
Licensing observed that 3 cats and 1 dog were not up to date on vaccinations.
