- The living room and kitchen were cluttered with bagged food on the floor of the kitchen, laundry on the couch, and surfaces covered with paper, books, and other items.
- Primary Caregiver and Assistant caregiver on-site and counting in ratio did not have current CPR and First Aid training. Expired on 12/28/2024.
