Listed below are the non-compliances OBSERVED during a visit. The regulation, its description, the non-compliance observed according to the regulation and a plan of corrective action are all provided.
Licensing reviewed children's records, four children do not have Insurance notification forms. One child does not have a Compliance file notification form
Licensing observed provider place a one year old in an outside hammock, while provider collected documents for licensing, cleaned outside lunch area, used the restroom and while the other children finished playing outside. Child feel asleep while in the hammock.
The outside hammock is in poor condition, with wide gaps in-between the seated area, allowing the child's feet to dangle outside of the hammock.
Licensing observed provider change a wet diaper outside, on a fabric quilt laying on the patio concrete.
Licensing observed provider has not associated herself with her home daycare on the registry. Provider explains she does not have a CECPD account for herself, and has not documented the training she has received. Licensing discussed this during the last Monitoring visit 3/15/24, and it has not been completed.
Licensing observed provider's medication bottles and vitamins, near the edge of the kitchen counter; easily accessible to children.
A caregiver reported to Licensing on this date a previous incident where a criminal investigation occurred involving individuals living in the home, providing care, or assisting with children's care;
Any complaint investigation that rises to the level of abuse/neglect is referred to Oklahoma Human Services Child Welfare Services for screening and investigation. All other complaint allegations are investigated by Oklahoma Child Care Services. Only substantiated complaints are available for viewing online.
