Non Compliance Observed Licensing observed two children to not have medication permission forms for diaper rash cream. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Person in Charge removed the diaper rash cream from the classroom while Licensing was present. Person in Charge will have Medication Permission forms for children ready upon parent pick up to have parents sign them. In the future, medication permission forms will be filled out prior to use of medication and according to requirements. Regulation Description Medication permission. Signed parent permission for each medication is obtained, prior to administration, on an OKDHS or program form containing the same information. Plan to Correct Date 2026-02-26
CPR and first aid
Non Compliance Observed Licensing observed staff to not have current CPR/FA. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Person in Charge will call trainer at South Western Technology Center to get staff scheduled as soon as possible to get her up to date. In the future, staff will have CPR/FA completed according to requirements. Regulation Description CPR and first aid. Cardio-pulmonary resuscitation (CPR) and first aid certifications are age-appropriate for the children's ages accepted into care and are from approved sources listed on the OPDR website. In addition to CPR and first aid certification requirements for individuals in (A) through (B) of this subsection, other position specific CPR and first aid certification requirements are also met, per OAC 340:110-3-284.1 through 340:110-3-284.4.
