During the visit, one parent came and filled out the compliance file notification form. At pick up, the other child's parent will fill out the form. In the future, parents will fill out the form in the office during onboarding.

During the visit, the parent came and filled out the medical permission form. In the future, parents will be required to turn in any medication to the office prior to dropping children off in their classrooms. Teachers will also check children's items for any medication and give them to personnel in charge prior to use. Until permission forms are received, the medication will be labeled and stored in the office in the medication bin. Once parent permission is received, the medication will be allowed to be used in the classroom.

  • Infants are placed in the appropriate sleep equipment when sleeping. There are no blankets covering infants. There are no blankets in or on infant sleep equipment. Infants were observed being removed from equipment restricting movement in a timely manner.
