Non Compliance Observed Two (2) new employees were employed on 2/16/2026 & in March 2026 and personnel information form's were not submitted to Licensing within 2 weeks of employment. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Licensing received personnel employed 2/16/26 personnel form during visit. Professional development
Non Compliance Observed Licensing observed a personnel hired on 2/16/2026, that does not have ELCCT training and four (4) personnel that has not completed infection control training since 9/21/2024. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Personnel is enrolled in ELCCT and will complete by the end of the week. Maintenance of facility
Non Compliance Observed Licensing observed a large hole in the ceiling where a tile is missing and insulation can be seen. Rest equipment
Non Compliance Observed Licensing observed a one-year-old in a crib asleep, laying on top of two (2) mattresses, the sheet was only over the corners on the top mattress, and the top mattress was pulled up away from the bottom mattress. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct One-year-old was removed to a crib with a single firm and fitted mattress. Director will send Licensing documents of where mattresses came with the cribs. Director purchases single thicker mattress when available. Any child that sleeps in a crib will be on the single thicker mattress and/or one-year-old's will be place on a cot if needed. Regulation Description Rest equipment.
